Chronic Illness ManagemenT program
Level 1
Annual membership - Our best value. This membership includes all functional lab testing (two full panels a year). Two comparative analytical reports. with detailed and customized health improvement plans and follow-up health maintenance plan to determine progress.
One initial comprehensive medical history intake. Two 50-minute follow up teaching consults.
Six 30 min wellness coaching and maintenance virtual consults to keep you on track toward your health goals.
One 192 Food Sensitivity Panel to determine foods that may be contributing to inflammation and symptoms.
This level is ideal for those with more chronic and challenging health conditions. $2,500.
SUMMARY:

Level 2
Our second level is a six-month membership. Ideal for those who are symptomatic but with no underlying health condition - to address symtpoms and receive guidance on how to stay healthy.
Includes one comprehensive initial medical history intake (45-60 min). One full functionsl laboratory panel. One functional health report analysis with customized health improvement plan along with follow up teaching consult (60 min). Four monthly 30 minute coaching and maintenance virtual visits to keep you on track. $850.
SUMMARY:

4 Thirty-minute follow up or coaching sessions as selected
Level 3 - Everything included in level 2 except the coaching sessions: $550
These packages establish you as a clinic patient.
A La Carte Fees:
Once you are an established patient by purchasing any of the above level packages, follow-up appointment fees are as follows:
Within 6 months of seeing the doctor $75 (if you have external labs you would like Dr. Moreira to go over with you, then the fee is $100)
After 6 months of not seeing the doctor $100 (50-minutes - you will need to complete a progress questionnaire)
After 12 months of not seeing the doctor, you are considered a new patient and may re-establish yourself as a patient of the clinic by purchasing one of the above packages.
